Practice Policies
It is our policy at Intelligent Health LLC, to reserve a patient's appointment time. We do not "double-book" patient appointments.
You will be charged $200 for missed appointments, e.g. appointments that are not cancelled 24 business hours in advance (i.e. for Monday appointments, you must cancel by the appropriate time the preceding Friday) and appointments that you are late by 50% of the appointment time (i.e. late by 15 minutes for a 30 minute appointment). If you miss or “late” cancel two consecutive sessions, your provider may discharge you from their service.
Charges for "missed" or "late" appointments are not covered by your insurance and are due and payable prior to any further appointments. Please note that such charges include the amount normally covered by the insurance company in addition to the copay amount.
The policy of Intelligent Health is that the financial fees for patient appointments must be paid in full by the patient or the Financially-Responsible Party(FRP) before patient's scheduled appointment or at the time of visit. If payment is not received, your credit card will be charged at the time of the appointment. All major debit/credit cards accepted. You may also use your flex and health savings account (HSA). As per policy and for your convenience, we ask you to store a debit/credit card with the secure client portal upon intake. You may choose to use your credit card on file regularly or as a backup.
It is your responsibility to verify your out-of-network coverage, benefits, limits, and exclusions for mental health services with your insurance company prior to your first appointment.
We follow HIPAA guidelines for your protection and you have the right to your medical information. We are required by law to maintain the privacy of, and provide individuals with, this notice of our legal duties and privacy practices with respect to protected health information.
